Neck exercises: 5 home rehab exercises with Tamil names, doses and steps

Hours on a phone, a laptop on the lap and a two-wheeler commute give India a neck-pain epidemic, and cervical spondylosis shows up on X-rays of most people over forty whether or not it hurts. Treatment is simple and repetitive: correct the forward-head posture, loosen the tight side muscles, keep the neck turning and build endurance in the deep neck muscles. These five exercises do that, and all of them can be done at a desk, which is where most neck pain starts.

Exercise 1 · Sitting

Chin tuck · தாடை உள்ளிழுத்தல்

Corrects the forward-head posture of phone and desk work by activating the deep neck flexors. The single most-prescribed exercise for mechanical neck pain and cervicogenic headache.

How to perform it

  1. Sit tall, looking straight ahead.
  2. Glide the chin straight back, making a double chin, without tilting the head up or down.
  3. Hold 5 seconds; feel the stretch at the top of the neck.
  4. Release. Do a set every hour at the desk.
Take care

It is a small movement; a finger on the chin can guide the first few reps.

Lengthens the upper trapezius and side neck muscles that tighten with stress and shoulder-hiking. Relieves the band of pain between neck and shoulder.

How to perform it

  1. Sit on one hand to anchor that shoulder down.
  2. Tilt the head so the ear moves towards the opposite shoulder.
  3. Add gentle pressure with the free hand if needed; hold 20 seconds.
  4. Return to centre and repeat on the other side.

Keep the nose pointing forward; do not rotate.

Restores the turning range needed to check mirrors while driving and to look over the shoulder. Done slowly, it also eases a stiff neck after sleeping awkwardly.

How to perform it

  1. Sit tall with the shoulders relaxed.
  2. Turn the head slowly to look over one shoulder as far as comfortable.
  3. Hold 3 seconds.
  4. Return to centre and turn the other way.
Take care

Dizziness or visual disturbance on turning needs assessment before continuing.

Exercise 4 · Sitting

Isometric neck press · கழுத்து ஐசோமெட்ரிக் அழுத்தம்

Strengthens the neck in all four directions without moving it, so it is safe even when movement is painful. Builds the endurance the neck needs to hold the head up through a work day.

How to perform it

  1. Place a palm on the forehead; press the head forward into the hand without letting it move.
  2. Hold 5 seconds, then release.
  3. Repeat with the hand on the back of the head, then on each side.
  4. Press at about 30 % effort, breathing normally.
Take care

Avoid in the first days after whiplash unless your physio says otherwise.

Shoulder shrug and roll · தோள் உயர்த்தி சுழற்றுதல்

Loosens the shoulder girdle, which carries much of the tension in neck pain, and improves blood flow during long computer sessions.

How to perform it

  1. Lift both shoulders up towards the ears.
  2. Roll them back, squeezing the shoulder blades together.
  3. Drop them down and relax.
  4. Reverse the direction for the second set.

Keep the movement slow and large; no jerks.

How to progress neck exercises, phase by phase

A neck programme is not one fixed list. The same 5 exercises are introduced in stages, and the dose rises only when the current stage is easy and pain settles within 24 hours. This is the usual order a physiotherapist follows:

Arm numbness, hand clumsiness, unsteady walking or dizziness on turning need a doctor's or physiotherapist's assessment before any of these phases.

How often, and how hard?

As a rule of thumb, mobility and range-of-motion exercises are done little and often (2–4 short sessions a day), while strengthening is done once a day or on alternate days with full sets. Mild discomfort up to 3–4 out of 10 that settles by the next morning is acceptable; sharper pain, new swelling or pain that spreads means drop back a phase and tell your physiotherapist.

Neck exercise questions

Is cervical spondylosis on my X-ray the cause of my pain?

Not necessarily. Age-related changes appear on the X-rays of most people over forty, with or without pain. Treatment is based on how your neck moves and what provokes it, not on the X-ray, and exercise helps regardless of what the film shows.

Which pillow should I use?

One that keeps the neck level with the spine when you lie on your side, usually a medium-height pillow of firm foam or a contoured one. Avoid very high or very soft pillows. A towel roll inside the pillowcase under the neck helps some people.

Can neck exercises cure my headaches?

If the headache starts at the base of the skull, is one-sided and comes with neck stiffness, it is probably cervicogenic and chin tucks plus upper-neck mobility often reduce it. Migraine and tension headaches have other triggers; your physio can help tell them apart.

When should I worry about neck pain?

See a doctor promptly for pain with arm weakness, numbness or tingling that does not settle, clumsiness of the hands, unsteady walking, dizziness on turning, or neck pain after an accident or a fall.

How long until the pain goes?

Most mechanical neck pain settles within 2–6 weeks with exercises done daily and posture changes at work. Flare-ups are common; keeping up the chin tucks and stretches after recovery is the best prevention.
