What is a home exercise program app?

A home exercise program (HEP) app is software a physiotherapist uses to prescribe exercises for the patient to do between clinic visits. The physio picks exercises from a library, sets the dose (sets, repetitions, hold time, how many times a day) and sends the plan to the patient's phone. A good HEP app also tells the physio whether the patient actually did the exercises, because a plan that is not followed does not work.

Most HEP apps used worldwide, such as Physitrack's PhysiApp, ask the patient to download an app and log in. That works for many patients in Europe or Australia. In India, a 68-year-old after a knee replacement, a farmer with a frozen shoulder or a mother with postpartum back pain is far more likely to open a WhatsApp message than install an app, and far more likely to follow instructions in Tamil or Telugu than an English handout. Why do patients stop doing their home exercises?

Physiotherapists in India see the same pattern: the patient leaves the clinic motivated, does the exercises for two or three days and then stops. The common reasons are simple and fixable:

Privacy and consent

Plans go only to the patient's registered number through the official WhatsApp Business API. The clinic records the patient's consent at registration (which helps it meet the DPDP Act 2023) and can switch WhatsApp off for a patient at any time. Done marks and 0–10 pain scores are stored per day against the plan in the clinic's workspace in India; staff of that clinic can see them, and they reach a referring doctor only in a progress letter the physio reviews and sends with the patient's consent. How do I know if a patient is doing the exercises?

Can patients who prefer paper still use it?
