ABHA and ABDM for physio clinics: ready in sandbox, honest about the rest

More patients now carry an ABHA number, and some referring hospitals ask for ABDM-linked records. Physio Clinic Software stores ABHA details, checks their format, simulates care-context linking and exports the physiotherapy record as a FHIR R4 bundle, so you can see exactly what would be shared before live linking is switched on.

In short: Physio Clinic Software records a patient's 14-digit ABHA number and ABHA address, format-checks them, simulates linking the physiotherapy care context in sandbox mode and exports the record (patient, completed visits, outcome scores and signed SOAP notes) as a FHIR R4 JSON bundle. Live ABDM linking is not active: it needs the clinic's Health Facility Registry ID, HPR IDs for therapists and ABDM gateway credentials after certification.

What works today (sandbox)

  • ABHA number (14 digits, shown as 91-1234-5678-9012) and ABHA address (name@abdm) stored on the patient record with format checks
  • "Link care context" in sandbox mode: details are checked and stored and a sandbox reference is logged; nothing is sent to ABDM
  • FHIR R4 export of the physiotherapy record: patient, completed encounters, outcome scores (NPRS coded with LOINC) and signed SOAP notes
  • Every ABHA action written to the audit log with user and time

What live ABDM linking still needs

Live linking under ABDM milestones M1 to M3 needs three things that only the clinic and the National Health Authority can provide: the clinic's Health Facility Registry (HFR) ID, Healthcare Professionals Registry (HPR) IDs for the physiotherapists, and gateway credentials issued after ABDM sandbox testing and certification.

We will switch on live ABHA verification, care-context linking and consent-based record sharing per clinic once those exist, and we will say so on this page. Until then nothing here is described as ABDM-certified or ABDM-compliant.

Is Physio Clinic Software ABDM-certified?

No. ABHA details, care-context linking and FHIR export work in sandbox mode. Live linking needs your clinic's HFR ID, HPR IDs and ABDM gateway credentials after certification, and we will only describe it as live once that is done.

Do I need ABDM to use Physio Clinic Software?

No. ABHA is optional for physiotherapy clinics today. Record ABHA details for patients who share them and use the FHIR export when a referring hospital asks for structured records.

What does the FHIR export contain?

A FHIR R4 collection bundle with the Patient (including ABHA identifiers if recorded), each completed visit as an Encounter, outcome scores as Observations and signed SOAP notes as Compositions. It is tagged as a sandbox export.

Is the patient's consent needed?

Yes. Record the patient's consent before storing ABHA details or sharing records. Under ABDM, live sharing will also require the patient's consent artefact through their ABHA app.
